INITIAL EVALUATIONS
- Evaluations can be scheduled by phone, however, if it is necessary to cancel, please provide at least 24 hours advance notice. There will be a $57.50 charge if less notice is given.
- We must receive the complete development history, signed policy statement, and any other required forms at least 48 hours prior to your child’s evaluation appointment. You may return the forms by mail or fax. We need these forms in order to adequately plan and prepare for your child’s evaluation.
- Please send any previous evaluations conducted by your child’s school district or other private agencies that may be pertinent to our evaluation and treatment.
THERAPY SESSIONS
- Parents are encouraged to sit in and observe their child’s therapy sessions. There is also a waiting room where parents or caregivers are welcome to wait and sit.
- If parents or caregivers would like to leave the building during the child’s therapy session, they must return at least 10 minutes prior to the end of the session. They must also be sure that their child’s therapist has a cell phone number and that they will be accessible by cell phone should an emergency occur.
- Therapy sessions are generally one-hour, 50 minutes of which is direct therapy time. The final 10 minutes are reserved for the therapist to write and review the child’s homework with parents and/or caregivers.
- Please be sure that you notify your therapist prior to the 50 minute mark if you have additional questions or items to discuss, so that that time can be allotted for that as well. We generally have a clients scheduled after you leave, so all discussions need to be complete during the session time.
- Please try to be on time for your child’s appointment. If you are late arriving, the session still needs to conclude on time so that the rest of the day’s schedule can remain on track. You will be billed for the full session.
ATTENDANCE
Regular attendance is essential for your child’s progress, therefore, a 75% attendance rate is required. We reserve the right to terminate services for any client falling below that percentage.
CANCELLATION POLICY
- If you have to cancel your child’s therapy appointment, please allow at least 24 hours notice. If you do not cancel within 24 hours, you will be billed 100% of the billable rate ($115.00).
- If you NO SHOW to an appointment you are billed 100% of the billable rate.
- Often parents do not know if a child is sick until the morning of the appointment. If this is the case, there will be no charge as long as you call first thing in the morning of the appointment day. We have an answering machine where you can leave a message.
PAYMENT
Parents are financially responsible for payment of service. Sessions will be billed at the end of the month. If you prefer to pay after each session, please let Sarah Paulson know ahead of time and arrangement will be made. Payment is due in the office by the 15th of the month. Payments not received by the 25th of the month will receive a 10% late fee.
INSURANCE
Kids Connecting does not bill insurance companies directly. Parents are responsible for paying the bill in full each month, unless otherwise specified. We bill at the end of the month and the payment is due in full by the 15th. We accept cash or check. On the bill will be insurance codes that insurance companies require to process a claim. The insurance company will then reimburse you if the service is covered.
REPORT AND MEETINGS
Please provide at least 2 weeks notice when requesting reports or attendance to any meetings.
